You certify that all information you provide is true, complete, and accurate to the best of your knowledge. You understand that Insurance Partners will rely on your information when evaluating, underwriting, quoting, issuing, or declining coverage. You agree to promptly notify us of any material changes to information you have submitted prior to policy issuance.
You authorize Phil Pavarini Insurance Agent, PestControlLiability.com, and any Insurance Partner to verify all information you provide and to obtain consumer and business credit reports, background information, and financial data for underwriting, renewal, claims investigation, compliance, and fraud prevention purposes. You authorize banks, courts, licensing boards, employers, and other third parties to release relevant information and release us and such parties from liability arising from lawful disclosure or use of such information.
You acknowledge that insurance premiums, service fees, and related charges are generally earned upon issuance of a policy and are non-refundable once issued, except where required by applicable law or the terms of the issuing carrier. Renewal premiums and ongoing obligations may continue until written evidence of cancellation or termination is accepted by the issuing carrier.
Phil Pavarini Insurance Agent acts solely as a referral intermediary in connection with this Site. All underwriting decisions, policy terms, coverage determinations, premium calculations, and claims handling are made solely by the applicable Insurance Partner. Any person who knowingly provides false, incomplete, or misleading information to induce the issuance of an insurance policy commits insurance fraud and may be subject to criminal prosecution, civil penalties, fines, and imprisonment under applicable federal and state law.
